​New CDL Medical Exam Forms Take Effect April 20

As of April 20, 2016, only the new Medical Examiner’s Certificate (Form MCSA-5876) issued by certified medical providers will be accepted as proof of physical qualification for exams performed on or after April 20.

The Oregon
DMV will continue to accept other medical certificate forms after April 20 only if the examination was performed before April 20, 2016.
The new medical certificate form is available only to medical professionals approved for the National Registry, so Oregon DMV will no longer provide blank forms to professionals who provide CDL-related medical examinations.

FMCSA regulations require CDL holders to maintain a current valid medical certification on file with their state’s DMV in order to maintain their commercial driving privileges. Since May 21, 2014, the examinations must be performed by medical professionals certified by FMCSA and listed on the National Registry of Certified Medical Examiners.
